To insure that your child is not held out from participation, all forms must be received by Wednesday, July 14th, 2013 for processing. All forms except the physical and health history (parent permission) are to be submitted using the Genesis Parent Portal. Physical and parent permission forms are to be turned in to the school nurse by the deadline. Access to Parent Portal TBA.

PLEASE READ THE FOLLOWING CAREFULLY TO ENSURE YOUR CHILD'S PARTICIPATION IN ATHLETICS FOR THE 2012 -  2013 SCHOOL YEAR.
 
In order to participate in athletics the state requires students to submit complete physical forms and permission (health/history) slips. The required forms may be accessed below. Form A is the health/history form. It should be completed by a parent/guardian. Form B is the physical form. It must be completed by a physician. All forms must be submitted to the school nurse complete. Incomplete forms will not be accepted. Any incomplete forms will be returned to you - at the expense of your child's athletic participation.  This also applies to 9th grade and transfer students in regard to physical education eligibility. Computer printouts will be accepted only for immunization records.  Should your physician question this, please refer to State Code 6A:16-2.2, specifically, h1-6.
The Pascack Valley Regional District rrequires all athletes (and parents/guardians) to sign a sportsmanship contract and a substance abuse and tobacco policy form. Athletes are not permitted to participate without signing and returning the contracts to the director of athletics. (This is the fourth form listed below.)
The NJSIAA has passed legislation requiring all student/athletes and their parents/guardians to sign a "steroid testing permission slip." The link for the permission slip is listed below as is the NJSIAA list of banned substances. Failure to sign the form will render the student athlete to be ineligible for competition.
 
Please note: All athletic participants must be cleared before they can begin practice. That includes all medical forms, parent permission form, NJSIAA Steroid Policy Agreement, State Concussion Acknowledgement Form, and PHHS Sportsmanship Contract. All forms must be received by Wednesday, July 24th 2013.
